How To Register Your Advance Directive

It takes a little time, but registering your advance directives saves time and energy for your medical providers and loved ones. It’s also easy to do:

  1. Prepare one or more of the documents listed above. Make sure the documents are witnessed or notarized, if necessary.
  2. Print the Registration Agreement from the Advance Directive Registry website.
  3. Complete the form, following the instructions carefully.
  4. Make a legible copy of your advance directive(s) and paper clip to the registration agreement.
  5. Sign and date the Registration Agreement, then submit by mail or in person:
